1) Church Data Form

Please select your church from the pull-down menu, then hit the "Go" button.

Session Review Checklist Form - This will be handled differently this year. We will review 2018 Session Minutes by meeting face-to-face and reading each others' minutes. The Stated Clerk will send more information, including a revised check list to each Clerk of Session by February 1, 2019.

 

Please print a copy of each form, fill-in and/or correct the requested information, and return to the presbytery office by February 1, 2019.

The deadline for submission is: February 2, 2019.

Clerk's Annual Questionnaire
In mid-October, GA emailed each clerk or congregation a personal link to the Clerk's Annual Questionnaire. The deadline for submission was: November 15, 2018.

To download your church's
data form:

Please select your church from the pull-down menu, then hit the "Go" button. You will be prompted for a username and password (This will be mailed to the clerk of session).

The password will be in effect until you close your browser window. If you close your browser window you will be prompted to re-enter the username and password.

All other forms on this page are the same for each church and do not require a username or password.

Teaching Elder and Commissioned Ruling Elder Forms:
Annual reporting forms will be mailed in late January/early February. If you cannot locate your forms, please go to the Teaching Elders & CRE Forms page for new forms.
