Please print a copy of each form, fill-in and/or correct the requested information, and return to the presbytery office by February 1, 2017.

Session Annual Statistical Report Workbook

Starting December 1, 2017, you will be able to enter your church's statistical data using this link: Statistical Reporting

Clerk's Annual Questionnaire
GA will be emailing each clerk or congregation a personal link to the Clerk's Annual Questionnaire via Survey Gizmo. The CAQ will be sent sent to clerks of session mid-October. The deadline for submission is: December 12, 2017.

To download your church's
data form:

Please select your church from the pull-down menu, then hit the "Go" button. You will be prompted for a username and password (This will be mailed to the clerk of session).

The password will be in effect until you close your browser window. If you close your browser window you will be prompted to re-enter the username and password.

All other forms on this page are the same for each church and do not require a username or password.
